This is a form of magic in which spells can be selectively applied
depending on whether one is related to the caster. Its only real use is
to solve plot problems of the "why didn't anyone else do that already" variety, and to reveal the Regina/Zelena relationship.
When Neal whirls an old cane of Rumple's, it unlocks a cloaking spell that had been hiding a cabinet of magical items (3.03).
Regina seals the door into her palace through the tunnels with blood
magic (3.13), and used what she calls a "blood lock" on her own office
in Storybrooke when they set the trap for Zelena there.
While
looking for a way to activate the magic ink that will not require
turning Emma to darkness (4.20), Rumple instructs Isaac to look for
information on blood magic.
Cora
has the box in which she keeps her wand sealed with blood magic (5.19),
which is how Regina and Zelena discover they are sisters.
